 | Pay the Registration Fee: This is
presently $100.00 and is payable at the time you submit your Registration
Application. For the summer session only (July and August,) the Registration
Fee is $25.00. This fee is non-refundable unless we are unable to provide space
for your child at Country Park. |
 | Remit the Security Deposit: One week's tuition which will
cover the last week's care under the terms of your Contract. This deposit is due
at the time you submit your Registration Application and it will be
refunded if we are unable to provide space for your child at Country Park. |
 | Complete the Authorization Form:
This form specifies those individuals who are authorized to pick up your child
(other than the child's parents or guardians) and gives us
authorization for your child to receive emergency medical care should you and
all of the emergency contact persons you've specified be unavailable. This
document is part of our Parents Guide which you will receive when you
take your tour. |
 | Submit a Medical Report: You must
submit a completed New York State Medical Report for Child in Day Care
for your child, signed by a physician. This form is also part of the Parents
Guide which you will receive when you arrive for your tour. |
